- Improved Resource Allocation and Planning: Insights to guide local healthcare providers in making informed decisions about staffing, structure, and service delivery.
Student Involvement:
Data Collection and Integration:
- Collect data from key sources such as Hospice Muskoka, the District of Muskoka, Simcoe Muskoka District Health Unit, and Statistics Canada.
- Gather information through interviews with healthcare providers and local stakeholders to understand patient flow dynamics and challenges in care delivery.
Data Analysis:
- Complete assessment of patient flow data, identify trends and barriers.
- Use statistical tools and models to identify patterns in healthcare use.
Projection Modeling:
- Develop a demographic-based projection model using current population data, aging trends, and health status from Hospice Muskoka, the District of Muskoka, Simcoe Muskoka District Health Unit, and Statistics Canada.
- Include potential variables (e.g., changes in healthcare structure, advancements) that could affect patient flow and care needs.
Mapping the Current and Projected Healthcare Needs:
- Create visual maps that show patient flow within the Muskoka region, along with areas of high demand for specific healthcare services.
- Develop a dynamic, interactive tool to allow stakeholders to visualize both current and projected data.
Develop Visual Diagram Flow Pathways
- Unattached to PCP → Referral to Palliative Care → Care Plan → Primary Care or Community Resources → Follow-up & Ongoing Support
- Hospital Admission → Palliative Care Assessment → Symptom Management → Care Plan/Family Support → Transition to Home/Hospice → Discharge & Follow-Up
- Home Care → Referral to Home Palliative Care → Symptom Management → Caregiver Training/Education → In Home Follow-ups → Hospice Referral (if needed)
- Hospice Care → Hospice Admission → Comfort & Symptom Management → Family Support → End-of-Life Care → Post-Death Support
Report Creation and Presentation:
- Prepare a detailed report outlining the findings, with key understandings into current and projected healthcare needs, focusing on hospice and palliative care services.
- Provide recommendations for local healthcare providers based on findings.
Stakeholder Engagement and Feedback:
- Present initial findings and projections to local healthcare stakeholders for feedback and improvement.
Resources Required (if any):
Hospice Muskoka MEDITECH EXPANSE EMAR, Hospice Muskoka KPIs, data from the District of Muskoka, Simcoe Muskoka District Health Unit, and Statistics Canada.
Support Offered to Students (i.e. weekly meetings, mentorship, etc.):
- Weekly meetings via MS Teams or in person at Andy’s House for 1 hour each week can be requested if support is required by students completing Capstone Project.
